One Crown Services offers a range of care options for seniors, adults and children with disabilities. Don’t think of our services as one-size-fits all. We know everyone’s needs are different, so our care plans are uniquely tailored to each individual. One Crown Services’ dedicated caregivers and staff will help you or your loved one live fully. Our trusted and reliable caregivers are trained to assist with all types of needs and to deliver an exceptional care experience in a variety of settings: at home, in an independent or assisted living community, or elsewhere.
Assessment of Service Needs
In order to assess the needs of individuals served, it will be the practice of One Crown Services to meet with the person served and his or her team to discuss specific needs, complete a preference assessment and intake packet. The preference assessment will help take a person-centered approach to determine the needs, goals, and preferences of the individual served. The purpose of the intake packet will be to obtain personal, medical, and social history and outline the personalized services requested by the person and involved parties.
Development & Implementation
After meeting with the person served and his or her team to include guardian(s), Service Coordinator, and other involved parties, supports including residential habilitation support plans will be outlined. The development and implementation will be based on the person’s PCP and habilitation services will be written according to the decision of the team by the QIDP with approval of the Service Coordinator.
Behavioral Support
To provide behavioral support and services, it will be the practice of One Crown Services to utilize positive behavioral supports in everyday interactions. In addition, behavioral data will be collected as deemed necessary for the purpose of presenting behavior to the team and behavioral professionals (BCBA, psychiatrists, clinical staff etc.). The use of additional behavioral training will be provided to direct support staff to deliver service.
Resources & Support Networks
Resources and support networks will be evaluated upon intake by interviewing the person served and his or her team via preference assessment and social assessment. Continuous communication with the person will assess if the person’s needs are met and there is a satisfaction of services. Once resources are identified, a specific plan will be made for the person on accessing the resources and support. We will utilize previous contacts and known community resources to help offer the person options that are available. Access to resources will be obtained and discussed with the person prior to proceeding or setting up other services.

Personal Outcomes
One Crown Services aim to facilitate personal choice and personal outcomes as defined by the person. Information collected upon intake will define what is important to the person. Services will revolve around preference to include but is not limited to: plans, daily activities, interactions, staffing, training, and frequency of activities and contact with others. An evaluation of outcomes will be reviewed regularly as the interests of people change. Education will be regularly provided to each person about their personal rights and responsibilities.
Facilitating Community Integration
It is a goal of ours to utilize the choices and preferences of the individual served to identify housing, services, resources and activities in the local community for their personal benefit. This includes all community services and establishments including banks, post offices, hospitals, retail stores, recreational facilities, etc. as needed by the individual served. The natural participation of these activities supports community integration. It will be the practice of the agency to provide staff training in this area and resources such as community event calendars and scheduled activities available for supported people to choose from.
Health & Medical Related Support
One Crown Services will facilitate continuous care with previous medical providers or set up new providers based on professional referrals or needs. Follow-up for health and medical related supports will be conducted regularly to include appointment scheduling and physician visits, medical record-keeping, and safely obtaining and taking medications. All other aspects of health for the individual served can be identified specifically by the team and by medical professionals. Ongoing tracking of each person’s medical records, medical history and medical needs outlined by annual physical exams or nursing assessments will be recorded. The agency will use HIPAA-compliant and take measures for the use of PHI and other health related data.
Transportation Services
Transportation will be provided by the members of this agency as it is necessary and possible in accordance with state laws and regulations, insurance policies, and as deemed safe to do so. If it is not possible to provide transportation in agency-approved vehicles, other arrangements will be made as appropriate utilizing public transportation such at RTC, RTC Access or shared rides.
